Once you click "Pre-Register", you will be asked for your personal information throughout the next couple of pages.  When this is completed, please allow up to five minutes to receive an email with directions what your username is, and how to create a password.  You will be presented with several consents that need to be acknowledged prior to approval.  Upon approval, you will be directed to your personal patient portal.
